6. A Comparison of Service Quality in Teaching and Non-teaching Hospitals: The Gap Analysis

Abstract

Introduction

: Service quality is a judgment that the customer compares his/her expectations by this judgment, with the perception of the received services. The purpose of this study was to compare service quality in teaching hospitals with non-teaching hospitals in Kerman city.

Methods : This cross-sectional study has been done on 166 hospitalized patients and the standard SERVQUAL questionnaire with an approved validity and reliability was used. Data analysis was performed using SPSS software version 19.

Results : The results show that the service quality from the patients’ perception in non-teaching hospitals is more than the teaching hospitals (p=0.002). Most of the service quality gap is in the tangibility dimensions and also in providing amenities for patients and their companions.

Conclusion : Teaching setting of the hospital typically can cause poor subjectivity in patients about the quality of services provided and it is recommended that authorities identify factors causing these kind of thinking and solve them. Also the negative scores indicate a need for improvement in all aspects, especially the tangibility dimensions having the maximum gap.

Data were collected through two questionnaires including Beugre OJ scale and the OCB questionnaire made based on Organ dimensions of citizenship behavior (conscientiousness, altruism, courtesy, sportsmanship, civic virtue). The Structural Equations Modeling (SEM) was used to determine the effect of the variables. Data analysis method was SEM and the softwares SPSS and Amos have been used. Distributive justice had the greatest impact on civic virtue and sportsmanship and the final model of the impact of distributive justice on organizational citizenship behavior has been approved. Based on the findings of this study, if the hospitals increase the sense of justice among their nurses, the nurses will pay more attention to concepts such as organizational citizenship behavior too.

The effective compatible in-service training: a structured review of8.  literature

ABSTRACT

Background & Aim:  Undoubtedly, the knowledge and skills of the employees lead to the survival of the organization. The purpose of this study is to investigate the important issues in designation and implementation of staff in-service training models to determine a framework for designing the efficient nursing continuing education models.
Methods & Materials:  This study is a structured review based on Cochrane five-step protocols, in which18 qualitative articles from six English, and three Persian databases were extracted and analyzed using seven-step process of Noblit and Hare method in meta-synthesis without any limit of time. The English databases included “Science Direct, PubMed, Google Scholar, EBSCO, Proquest, and SAGE Journals” and Farsi databases were “Noormags, Iran Doc, and SID.” A standardized protocol along with specified criteria was used to study selection.
Results:  In the initial analysis of findings four themes were extracted: process for staff training, method for staff training, educational effectiveness, and specific model for staff training. The final analysis of findings showed that the models presented in the field of staff training can be generally divided into three categories: (a) Models that deal with the designation of processes and functions of staff training, (b) models that focus on the effectiveness of staff training, and (c) models that deal with both aspects.
Conclusion:  This study highlights the important role of effectiveness strategies as well as compatible training functions in designing the nursing staff in-service training models. In this way, the staffs motivate to participate actively in the organizational trainings.

9. Determination and priority of socioeconomic determinants of health in Iran: a9.  mixed methods study

Abstract:

The socioeconomic factors have crucial effects on health inequality. The unequal distribution of socioeconomic factors may lead to the health inequality. This study aimed to determine and give priority to the socioeconomic factors affecting health in Iran in the experts’ point of view. This study was a sequential mixed methods research conducted in quantitative and qualitative phases. The study population consisted of 16 policymakers and experts in the field of economic and social determinants of health. The purposive stratified non-random sampling method was employed. The qualitative data were collected in a semi-structured interview and giving priority to factors was conducted using multiple criteria decision making with the Technique for Order of Preference by Similarity to Ideal Solution (TOPSIS) method. The qualitative phase showed that the main socioeconomic factors determining health were the economic factors, income, education, sex, social class, and employment. The data analysis in TOPSIS technique showed that priority parameters were income, employment, and economic factors with coefficient of 0.66, 0.60, and 0.55, respectively. Design/methodology/approach In this qualitative study with content analysis approach, 20 hospital healthcare personnel participated as participants from the fields of medicine and nursing. Purposive random sampling and semi-structured interviews were used for data collection. Atlas.ti software version 5.2 was used for data analysis. Findings Four themes and 32 subthemes were identified by numerous revisions and combining the codes. The four main themes of sentinel events were: causes, incidence barriers, cause prevention solutions, and barriers' improvement solutions. Moreover, these main factors were related to these issues: staff and patients' education, communication, assessment, patients and their companions, employee rights, leadership, care continuum, human factors, physical environment, information management and medication use. Some solutions were also suggested according to these factors and a policy was recommended. Practical implications Hospital managers and authorities should try to find the main causes of sentinel events by periodical analysis to find ways to prevent them in the future, using logical and reasonable solutions.
